DO I NEED A REFERRAL TO BE SEEN FOR PHYSICAL THERAPY?

In the state of CA, you may go directly to a physical therapist for up to 12 visits or 45 days (whichever comes first), before seeing a physician or chiropractor for physical therapy treatment of an injury or specific condition. However, if it appears that you need care that is beyond the scope of physical therapy, we will refer you out to the appropriate practitioner at that time. Please note that physical therapists cannot legally make a medical diagnosis. Some insurance companies insurance companies require a diagnosis code to receive reimbursement so you may still want to see your MD or DC prior to starting PT if you are planning on submitting to your insurance.

DOES ASCEND BILL INSURANCE FOR PHYSICAL THERAPY SERVICES?

In order to provide a high standard of care, Ascend has chosen to be out of network provider and will not bill your insurance. However, we will provide the necessary superbill for you to submit to your insurance directly.

WHAT PAPERWORK DO I NEED TO FILL OUT PRIOR TO COMING FOR MY FIRST APPOINTMENT?

Any necessary paperwork will be emailed to directly the day you book the appointment. Please have all forms completed and submitted prior to your first appointment so your therapist can review it. The paperwork should take no more than 15 min to fill out. It is best to fill it out on a computer and not a smart phone. If you do not have email or access to a computer, please arrive 15 min early to complete the paperwork before your appointment.
